29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded, so as to prevent the operator from having any part of his body in the danger zone during the operating cycle:  a) Fabrication Shop:  On or about February 24, 2015, and at times prior thereto, employees were exposed to amputation hazards from the lack of a guard on the Amada Press Brake, model RGM2-8024, while bending sheet metal.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):   Fabrication Shop:  On or about February 24, 2015, and at times prior thereto, employees were exposed to the hazard of electrical shock from unguarded live wiring on the National Hydraulic Shear, model NH 7225, during cutting operations due to the lack of a lower front panel.
