29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300:     a) William LaRoque Installers, Inc. dba W.L. Installers, Inc. did not ensure detailed information was provided on the Log of Work-Related Injuries and illnesses, OSHA's Form 300, for the 2014 calendar year in columns (E) and (F) that clearly described where the event occurred and a description of the injury with the object/substance that directly injured the employee.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
