Where employees provide their own protective equipment, the employer did not assure its adequacy, including proper maintenance, and sanitation of such equipment.  (a)  BYU Student Housing:  On or about November 20, 2014, and at times prior thereto, employee-supplied anchorage connector (cross-arm strap) was frayed and not properly equipped with a connecting device (shock-absorbing device) and inadequate for field use. 29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards:  (a)  On or about November 20, 2014, and at times prior thereto, An employee was utilizing an employee-owned personal fall arrest with a rope grab system that was not equipped with a lanyard.  The equipment was frayed and required removal from service.  Employee demonstrated a lack of knowledge and/or the required skills and/or understanding of the personal protective equipment (PPE).  knowledge as how to properly inspect it, use and wear it.
