29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a. On or about February 25, 2015, on the ground below employees performing roofing work, an employee was working below the roof and while not wearing a hard hat, the employee was exposed to being struck by roofing materail and tools falling from the roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a. On a 1:12 pitch roof, employees conducting re-roofing operations were exposed to a 20 foot fall hazard in that they are not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder: a. On or about February 25, 2015, On the front of the building, the 24-foot extension ladder being used to access the roof did not have the ladder side rails extended to at least 3 feet (.9m) above the upper landing surface, exposing employees to a 20-foot fall hazard.
