29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate:    (a) On February 26, 2015, in the main indoor storage warehouse area, the employer did not ensure that the upper storage area was posted with a load limit sign showing the approved load limits.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:      (a) On February 26, 2015, at the facility located at 1411 E 19th St, Kansas City, MO., the employer did not ensure that all Powered Industrial Truck Operators were evaluated within the past three years. The last Performance Evaluation was conducted Sept. 28, 2010.     Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
