29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Press Room: Slitter Machine John Dusenbery (Model: 58842 Serial: 10000), On or about, March 3, 2015, and times prior thereto, employees were exposed to a caught in hazard while adjusting guides on a machine during operation near an unguarded rotating shaft with protruding lug guides.  (b) Press Room: Slitter Machine John Dusenbery (Model: 59571 Serial: 835 L-24), On or about, March 3, 2015, and times prior thereto, employees were exposed to a caught in hazard while adjusting guides on a machine during operation near an unguarded rotating shaft with protruding lug guides.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Press Room: Gemini S.12 Carint Machine (Model: Unknown Serial: Unknown): On or about March 3, 2015, employees were exposed to an electrical shock hazard and/or arc flash hazard from a 4 inch by 4 inch junction box that was mounted to the side of the machine and did not have a cover plate exposing conductors with a maximum rating of 20 kilovolts AC/ 30 Kilohertz/ 70 Amperes maximum current while employees use the light switch on the same side of the machine.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  (a) Press Room: On or about March 3, 2015, employees were exposed to an electrical shock hazard while using a Dorland electric 240 volt hoist (Model: 72R1ASI) to carry roll material for placement on machinery that had the flex conduit strain relief disconnected on the pendant side.  (b) Press Room: On or about March 3, 2015, employees were exposed to an electrical shock hazard while using a Dorland electric 240 volt hoist (Model: 72R1ASI) to carry roll material for placement on machinery that had the flex conduit strain relief disconnected on the in-line voltage side.
