29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Second floor top plate, new residential construction located at 2621 Parks Edge Drive, Smyrna, GA 30080: Employees were exposed to eye injuries while operating pneumatic Hitachi (Model - NR83A3S) 3-1/4-inch Framing Nail Guns at 120 PSI without protective eyewear.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Second floor top plate, new residential construction located at 2621 Parks Edge Drive, Smyrna, GA 30080: Employees were exposed to falls of approximately 21-feet, 4-inches  to the lower level while setting roof trusses from the top outside plates with no fall protection system in place.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  (a) Stairway inside structure, new residential construction located at 2621 Parks Edge Drive, Smyrna, GA 30080: Employees were exposed to falls of up to 6-feet, 5-inches while accessing the second floor which exposed employees to falls to the concrete surface below as a stair rail was not installed along the opens sides and edges.
