29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: Employees engaged in milling, material handling and maintenance activities at the following locations were exposed to fall hazards in that a swing gate was not provided at the following ladderway openings: A) At the east mezzanine in the mechanic shop, employees were exposed to fall hazards of approximately 7 feet from the mezzanine to the ground below in that ladderway was not guarded. B) At the boot pit, employees were exposed to fall hazards of approximately 10 feet from the boot pit entrance level to the lower level of the boot pit in that the ladderway was not guarded.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Employees engaged in cleaning and milling activities on the south side of the boot pit were exposed to fall hazards of approximately 5 feet from the walking/working surface to the lower level of the boot pit in that guard railing was not provided.
