29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  Employees engaged in milling, material handling and maintenance activities at the following locations were exposed to fall hazards in that a swing gate was not provided at the following ladderway openings:     A)  At the east mezzanine in the mechanic shop, employees were exposed to fall hazards of approximately 7 feet from the mezzanine to the ground below in that ladderway was not guarded.  B)  At the boot pit, employees were exposed to fall hazards of approximately 10 feet from the boot pit entrance level to the lower level of the boot pit in that the ladderway was not guarded.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replace:  Employees engaged in welding activities in the mechanic shop using the Miller Thunderbolt XL stick welder were exposed to electrical shock hazards in that the hot lead was damaged exposing bare conductors.
