OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to amputation and crush-by hazards from damaged dock leveling systems:   On or about February 26, 2015, employees receiving cargo were exposed to the following injuries and the employer did not remove damaged dock leveling systems from service, have the systems inspected by the manufacture's authorized representative, or have the system repaired as necessary before placing it back into service.:  a) Employees were exposed to crush-by injuries, broken bones, and amputations while manually lifting the lip of a defective, mechanical, fixed-type dock leveler at receiving dock #14.   b) Employees were exposed to crush-by hazards while driving a forklift onto the dock leveling system when it dropped below floor level at dock #14.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
