29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Warehouse, On or about March 04, 2015, employees were exposed to amputation hazards, when using the Hytrol conveyor serial #566163, which was not provided with guards on the pinch point locations on the top and bottom tail pulley ends of the conveyor, while loading and unloading air filter boxes.

29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  Air Filter Manufacturing Area, On or about March 04, 2015, employees were exposed to amputation hazards when using a Baileigh Industrial table saw, model #TS-1248P-36 with serial #122894, which was not provided with a guard over the cutting blade above the table, while cutting air condition air filters.
