29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  A.  At the workplace, in the warehouse, wood pallets were stored in unsecure tiers stacked approximately 13 feet in height and leaning, thereby exposing employees to a struck-by hazard from a collapse.

The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. 29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  a.
