29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):    On or about March 3, 2015, In-between the toolpusher, bunk and living quarter trailers; employees were exposed accidental contact with electrical equipment and trip hazards in the following instances:      a)    The ground electrode rod used for site bonding was not installed with the upper end of the electrode rod flush with or below ground level. The electrode rod end above ground and the grounding electrode conductor attachments were not protected against physical damage through accidental contact.       b)    The electrode rod/grounding electrode conductor attachments measured approximately 3-feet above the ground with the conductors sloping outward approximately 6-feet and downward to 8-inches above the ground exposing employees to trip hazards.
