29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling:  On or about March 4, 2015, at Big-E Rig 4, listed or labeled equipment was not installed and used in accordance with manufacturer instructions exposing employees to electrical shock hazards in the following instances;    a.     Behind the driller trailer, the 2800-gallon water tanks electric motor pump was wrapped using a heater tape to keep a nozzle from freezing; the heater tape was plugged into a 50-foot orange extension cord. Manufacturer instructions specifically states do not use with extension cords.    b.    Behind the driller trailer, the 2800-gallon water tanks electric motor pump was plugged into a 50-foot orange extension cord then daisy-chained using another 50-foot red/black extension cord which then plugged into the main site transformer. Manufacturer instructions specifically states do not plug one extension cord into another.

Oil Patch Rental trailer OPR 1162 (used by Directional Driller) was not grounded.    b.    Oil Patch Rental trailer OPR 1101 (used by Mud Engineer) was not grounded.

29 CFR 1910.305(e)(2): Switches, circuit breakers, and switchboards. Switches, circuit breakers, and switchboards installed in wet locations were not enclosed in weatherproof enclosures.  On or about March 4, 2015, at Big-E Rig 4, the employer did not ensure utilized switches, circuit breakers, and switchboards installed in wet locations were enclosed weatherproof enclosures, employees were exposed to electrical shock hazards in the following instances;    a.     In front of the Oil Patch Rental trailer OPR 1101 (used by Mud Engineer) the cover to the circuit breaker was missing.    b.    In front of the Oil Patch Rental trailer OPR 1101 (used by Mud Engineer) the cover to the two-120V outlet receptacle box was missing its weatherproof cover.
