29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use: On or about March 3, 2015, at the rig substructure, while welding together the base of the Blowout Preventer (BOP) one employee was observed using a Makita (Model No. 9557PB, Serial No. 042082K) angle grinder with a 4 ½ -inch abrasive grinding wheel attached to level out the welds; the angle grinder did not have guard and exposed the employee to struck-by hazards.

29 CFR 1926.403(e): Conductors were not spliced or joined with splicing devices designed for the use or by brazing, welding, or soldering with a fusible metal or alloy: On or about March 3, 2015, at the rig substructure, while welding together the base of the Blowout Preventer (BOP) one employee was observed using a Makita (Model No. 9557PB, Serial No. 042082K) angle grinder powered by a yellow extension cord that was not spliced by brazing, welding, or soldering with a fusible metal or alloy and was not covered with an insulation equivalent to that of the conductors. This condition exposed the employee to electrical shock hazards.
