Employee(s) observed gaining access to a walking/working surface in close proximity to          an unprotected edge without any means of fall protection, thereby exposing workers         to an approximate 13 ft fall hazard.          Violation observed on or about 02/11/2015          b. Employee(s) observed gaining access to a walking/working surface in close proximity           to an unprotected edge without any means of fall protection, thereby exposing           workers to an approximate 45 ft fall hazard. Employee(s) observed accessing an upper landing surface at an approximate height of               13 feet with a Werner Aluminum Extension Ladder that extended approximately 1 foot          above the upper landing.          Violation observed on or about 02/11/2015         b. Employee(s) observed accessing an upper landing surface at an approximate height of         45 feet with a Werner Aluminum Extension Ladder that extended approximately 1 foot         above the upper landing.
