29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    A)    Humanetics Innovative Solutions, Inc., located at 900 Denton Drive, Huron, OH 44839: On or about March 5, 2015, the employer failed to ensure that the open-sided floor of the loft, located above the maintenance room, was equipped with standard railing in accordance with 29 CFR 1910.23(e). Employees were exposed to a fall hazard of approximately 10 feet from the floor of the loft to the concrete floor below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Employees were exposed to injuries from unexpected start-up of the machine and energy release while electrical energy was not controlled prior to conducting work.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. An employee was exposed to 480 volts/3 phase electrical hazards while wiring the battery charger in the energized electrical panel, located in the maintenance room, without locking the panel out and without proper electrical personal protective equipment.      B)     Humanetics Innovative Solutions, Inc., located at 900 Denton Drive, Huron, OH 4839: On or about March 2, 2015, the employer failed to ensure that maintenance employees were provided with training on electrical safety-related work practices such as, but not limited to, electric shock hazards and protective equipment for the hands to protect against electrical hazards while working on or near energized or potentially energized electrical parts.  An employee was exposed to 110 volts electrical hazards while moving electrical conduit and an electrical cord in Materials Department without proper electrical personal protective equipment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees were exposed to caught-by and amputation hazards.      Humanetics Innovative Solutions, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.212(a)(1)), which was contained in OSHA Inspection # 313788630, Citation number 1, Item number 2a, and was affirmed as a final order on 09/30/2011, with respect to a workplace located at 900 Denton Drive, Huron, OH 44839.    No additional abatement information is needed for this item.
