29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, or were altered either by the addition of extra parts not provided by the manufacturer or by the elimination of parts: a) Warehouse - On or about 03/09/15 employees operated the Caterpillar 50 LPG-powered forklift which was modified with the removal of the front rack. The roller chains and the hydraulic hoses were exposed, and were not protected from damage by the load on the forks. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The hose connected to the LPG cylinder was hardened and cracked. The forklift was not removed from service until repaired, exposing employees to fire and explosion hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
