29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening. On or about 9 March 2015, and at times prior thereto, employees accessed an elevated platform to install seasonal umbrellas within the store in which the ladderway opening was not guarded, exposing the employees to a fall hazard of approximately 12 feet 6 inches to the concrete floor below.

29 CFR 1910.303(b)(7)(iv): There existed damaged parts that could have adversely affected the safe operation or mechanical strength of electrical equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. On or about 9 March 2015, and at times prior thereto, a four-plug outlet located within the Outdoor Lawn and Garden department was separated from the fixed conduit with the internally sheathed wires visible, exposing employees using the outlet to an electrical hazard of approximately 120 volts. At the time of the inspection, it was raining, and drops of water could be seen on the metal conduit.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. On or about 9 March 2015, and at times prior thereto, flexible cords were not protected from accidental damage, exposing the employees working within the area to an electrical hazard of approximately 120 volts. Within the Outdoor Lawn and Garden area, cashier stations 11-14 had electrical cords running through an open metal hole within the cashier's workstation.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy. On or about 9 March 2015, and at times prior thereto, within the Receiving office, an outlet missing the faceplate was being used by employees to connect power cords on various items, exposing the employees to an electrical hazard of approximately 120 volts. Lowe's Home Centers Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.0305(b)(2)(i), which was contained in OSHA inspection number 957022, citation number 02, item number 001 and was affirmed as a final order on 05/01/2014, with respect to a workplace located at 124 Route 101A, Suite 15, Amherst, NH 03031.
