29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:      a) Job site, 7401 McKnight Road, Pittsburgh, PA: On or about March 10, 2015, during steel erection activities, the employer did not ensure that an employee in an aerial lift used fall protection systems/devices where exposed to a fall hazard of approximately 15 feet.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:      a) Job site, 7401 McKnight Road, Pittsburgh, PA: On or about March 10, 2015, during steel erection activities, the employer did not ensure that an employee used fall protection systems/devices where exposed to a fall hazard of approximately 21 feet.
