29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:  A.)  eNNOVEA, LLC, 6100 West Ridge Road, Erie, PA 16506, maintenance shop area, on or about March 11, 2015 and times prior thereto:  A flexible electrical extension cord was powering equipment including, but not limited to, a parts washer pump.  The flexible electrical extension cord was damaged in that the outer jacket was frayed, and the inner insulation for the grounding conduction was frayed exposing bare copper.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected:  A.)  eNNOVEA, LLC, 6100 West Ridge Road, Erie, PA 16506, on or about March 11, 2015 and times prior thereto:  Employees perform service and maintenance activities on machinery that include, but are not limited to, injection molding machines.  Periodic inspections of the energy control procedures were not conducted with all authorized employees.  Authorized employees performing service and maintenance activities include, but are not limited to, those performing die setup.
