29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Construction site: On May 20, 2015 and at time prior, employees were exposed to head injuries from falling items such as but not limited to, hand tools and roofing materials while cleaning up directly under the roof while overhead work was being performed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) Construction site: On May 20, 2015 and at time prior, employees were exposed to a fall hazard of approximately 8'9 feet to the dirt ground below while removing shingles on a residential building with a 10/12 pitch roof.
