(a) In and around the Whatever It Takes Transmission Parts, Inc. warehouse - On or about March 11, 2015, employees operating the 1971 model Toyota powered industrial truck were exposed to injury due to the absence of a permanently attached seat, seatbelts, and an operational horn on the Toyota FGC15 powered industrial truck.
