29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    On or about 10 March, 2015, on the above addressed job site, two employees were installing plywood on a roof of a building under re-construction without the use of fall protection, exposing the employees to a fall hazard of up to approximately 7 feet and 11 inches to the floor below.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:    On or about March 10, 2015, two employees were accessing the low slope roof of a commercial building under re-construction with an A-frame ladder which was in its closed position leaned against the wall, exposing the employees to a fall hazard of approximately 8 feet in height.
