29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage.  On or about 3/12/15 3460 US Highway 341 in Fort Valley, Georgia: Employees were using a electrical hand router with several damage electrical cord sets plugged into an temporary outlets at 120 volts. The employees were exposed to and electrical hazards.
