29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: (a) BLM-Gila District, Tucson Field Office: On or about 12 March 2015 and at times prior thereto, a monitoring program was not developed and implemented. Employees are exposed to noise generated by gas-powered tools, shop tools and heavy equipment. In addition, Law Enforcement Rangers are exposed to high noise levels during firearms qualifications.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly: (a) BLM-Gila District, Tucson Field Office: On or about 12 March 2015 and at times prior thereto, monthly inspections of portable fire extinguishers were not conducted. The last annual inspection of the fire extinguishers was August 2014. BLM was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.157(e)(2), which was contained in OSHA inspection number 924903, citation number 2, item number 2 and was affirmed as a final order on 8 November 2013, with respect to the BLM-Ely District workplace located at 702 Industrial Way, Ely, NV 89301.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Shop, BLM-Gila District, Tucson Field Office: On or about 12 March 2015 and at times prior thereto, the rotating chuck of the "Northern Industrial Tools" drill press was not guarded. BLM was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 899070, citation number 1, item number 2 and was affirmed as a final order on 21 May 2013, with respect to the BLM-Eagle Lake Field Office workplace located at Fifth Street Shop, Susanville, CA 86130.
