29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  a) On or about 3/6/15 at the Quality Inn & Suites rehabilitation project located at 8200 Park Road, Batavia, New York; employees were using power tile cutting and mixing equipment plugged into extension cords. There were no ground fault circuit interrupters or assured equipment grounding conductor program in place.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:  a) On or about 3/6/15 at the Quality Inn & Suites rehabilitation project located at 8200 Park Road, Batavia, New York; throughout the work area(s) receptacles, switches and fixture boxes that were energized did not have covers or faceplates.  NO ABATEMENT CERTIFICATION REQUIRED
