29 CFR 1910.1048(h)(1)(i): All contact of the eyes and skin with liquids containing 1 percent or more formaldehyde was not prevented by the use of chemical protective clothing made of material impervious to formaldehyde and the use of other personal protective equipment such as goggles and face shields, as appropriate to the operation:    a)  St. Mary's Hospital & Medical Center, Inc. at 2635 N 7th St., Grand Junction, CO 81502:  On or before March 9th, 2015, employees performing transfers of 10 percent formalin solution and tissue into 10 percent formalin solution in and around the "train area" were exposed to chemical hazards as the employer did not ensure that employees were provided with and wore appropriate protective clothing to prevent dermal contact with spilled or splashed formalin solution.  Employees wore their scrubs and smocks during formalin transfers which were not chemical resistant.

29 CFR 1910.1048(i)(2): When employees' skin could have become splashed with solutions containing 1 percent or greater formaldehyde, the employer did not provide conveniently located quick drench showers and/or did not ensure that affected employees used these facilities immediately:    a)  St. Mary's Hospital & Medical Center, Inc. at 2635 N 7th St., Grand Junction, CO 81502:  On or before March 9th, 2015, employees performing transfers of 10 percent formalin solution and tissue into 10 percent formalin solution in and around the "train area" were exposed to hazards in the event of a spill or splash of formalin solution and the employer did not ensure that employees had immediate access to a conveniently located quick drench shower.  The nearest quick drench shower was not immediately accessible as it was approximately 100 feet away through two sets of doors.

29 CFR 1910.1048(d)(2): Employees who may have been exposed at or above the action level or STEL were not identified, and the exposure was not accurately determined for each identified employee:    a)  St. Mary's Hospital & Medical Center, Inc. at 2635 N 7th St., Grand Junction, CO 81502:  On or before March 9th, 2015, employees performing transfers of 10 percent formalin solution and tissue into 10 percent formalin solution in and around the "train area" and the employer had not identified such employees and verified that employee exposures to formaldehyde were not in excess of the action level or STEL.
