29 CFR 1910.106(a)(29): The safety can aka approved container not larger than 5 gallon capacity did not have a spring-closing lid and spout cover so designed that it will safely relieve internal pressure when subjected to fire exposure:     a) 806 Maple Avenue, Linden, NJ 07036. The employer allowed his employees to use an unapproved plastic gas can to fuel equipment, such as but not limited to chain saws when it did not have a spring-closing lid and spout cover.        Condition occurred on or about 03/13/15.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee was using appropriate eye and or face protection when exposed to eye and face hazards from flying particles and liquid chemicals:     a) 806 Maple Avenue, Linden, NJ 07036. The employer allowed his employees to operate chain saws while not wearing the appropriate eye and face protection exposing them to eye and face injuries from flying debris.      Condition occurred on or about 03/13/15.

The employer allowed his employees to work in areas where there was the potential for head injuries from falling objects when they were not wearing any head protection.     Condition occurred on or about 03/13/15.
