29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about March 9, 2015, at Highway 83 and Lions Club Road, Madison, Georgia: An employee performing pipe installation was exposed to struck-by hazards of rocks, soil, and dirt clods rolling into the approximately 8-feet wide by greater than 13-feet deep excavation from an approximately greater than 25-feet high spoil pile that started to roll to less than two feet from the edge and into the excavation.

29 CFR 1926.652(b)(2): Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart. On or about March 9, 2015, at Highway 83 and Lions Club Road, Madison, Georgia: An employee performing pipe installation was exposed to cave-in or struck-by hazard while working inside an approximately 8-feet wide by greater than 12-feet deep excavation when the employer did not shield the excavation at least 18-inches above the vertical side of the excavation.
