29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary.    On or about March 9, 2015, at Highway 83 and Lions Club Road, Madison, Georgia:    An employee performing pipe installation was exposed to struck-by hazards of rocks, soil, and dirt clods rolling into the approximately 8-feet wide by greater than 13-feet deep excavation from an approximately greater than 25-feet high spoil pile that started to roll to less than two feet from the edge and into the excavation.

On or about March 9, 2015, at Highway 83 and Lions Club Road, Madison, Georgia:    An employee performing pipe installation was exposed to cave-in or struck-by hazard while working inside an approximately 8-feet wide by greater than 12-feet deep excavation when the employer did not shield the excavation at least 18-inches above the vertical side of the excavation.

On or about March 9, 2015, at Georgia Highway 83 and Lions Club Road, Madison, Georgia:    An employee performing pipe installation was exposed to cave-in or struck-by hazard while working inside an approximately 8-feet wide by greater than 13-feet deep excavation when the employer did not ensure that the trench box was set within two feet of the bottom of the excavation.
