(b) L5 electrical cabinet rated at 208/110 volts had 5 missing circuit breakers on the right side of the cabinet allowing an approximately 5 inches by 4 inches opening. (c) The door for the top electrical box above the flood light electrical cabinet was left open exposing equipment / wiring rated at 120 volts. (d) The door for the flood light electrical cabinet was left open exposing electrical equipment / wiring rated at 120 volts. (e) The PH5 Cutler Hammer distribution panel rated at 480 volts / 50 amps had a 10 inch by 2 inch open section at the Sullair disconnect levers.
