29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, or by any of the following means identified in sub-paragraphs (A)through (D) of this paragraph:  Employee(s) working or walking through the electrical room were exposed to electrical hazards due to live parts not being guarded in the following areas:  (a) L5 electrical cabinet rated at 208/110 volts had 2 missing circuit breakers on the left side of the cabinet allowing an approximately 1 inch by 4 inches opening.  (b) L5 electrical cabinet rated at 208/110 volts had 5 missing circuit breakers on the right side of the cabinet allowing an approximately 5 inches by 4 inches opening.  (c) The door for the top electrical box above the flood light electrical cabinet was left open exposing equipment / wiring rated at 120 volts.  (d) The door for the flood light electrical cabinet was left open exposing electrical equipment / wiring rated at 120 volts.  (e) The PH5 Cutler Hammer distribution panel rated at 480 volts / 50 amps had a 10 inch by 2 inch open section at the Sullair disconnect levers.
