29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about March 17, 2015 at 766 North Broadway, Saratoga Springs, NY, 12866 employees of a subcontractor were exposed to a possible fall of 9 feet working taping and spackling sheetrock on the second floor of a residential addition near an opening for a stairway.  They were not protected by acceptable fall protection measures.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  a) On or about March 17, 2015 at the jobsite at 766 North Broadway, Saratoga Springs, NY, 12866 -  the employer did not have a written hazard communication program with respect to products being used in this workplace, such as but not limited to, propane (flammable) and accordingly employees were exposed to hazards of propane contact which if ignited could cause burns or in the liquid state could cause freeze burns.
