WAREHOUSE:  On or about March 18, 2015, the walkway at the end of a shelving unit near the washing machine was reduced to 15 inches by the placement of milk crates and boxes of items stored at the end of a shelving system and a parked powered industrial vehicle (Raymond Corporation Model 20-R40TT, Serial Number P20-81-20 265).

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.   WAREHOUSE AREA:  On or about March 20. 2015, compressed gas cylinders containing oxygen and acetylene were stored in the south end of the warehouse.  The cylinders were strapped to a support column with no barrier between the cylinders.  The cylinders were less than 20 feet apart and were located within 20 feet of combustible materials such as a wooden piano and pallets of cardboard boxes containing merchandise.

29 CFR 1910.303(g)(1)(i)(C): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of work space was not clear and did not extend from the grade, floor, or platform to the height required by paragraph (g)(1)(vi) of 29 CFR 1910.303:  WAREHOUSE AREA:  On or about March 18, 2015, transformers were installed immediately below the electrical panels thereby blocking access to the panels in the following locations:  A.  Northeast Corner of Building (Photo DSCN3394):  An electrical panel located in the northeast corner of the building was blocked by a Hevi-Duty transformer which extended 19 inches in front of the panel.    B.  Northeast Corner of Large Warehouse Room (Photo DSCN3379):  Access to two electrical panels labeled "240V 30" and "240/120V 10" was blocked by two transformers and a charger for powered industrial trucks.
