29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: a) On or about 03/17/2015, workers building an addition were exposed to a 120 volt electrical shock from an ungrounded heavy duty extension cord and an uncovered outlet box which were not provided with either an assured equipment grounding conductor program or with ground fault circuit interrupter circuitry. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: a) Addition On or about 03/17/2015, workers were exposed to 120 volt live parts while plugging an extension cord into an outlet box which had not been provided with a face plate or cover. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: a) Addition On or about 03/17/2015, workers were exposed to a 120 volt shock hazard while using a Makita portable circular saw (serial # 073358) to saw lumber whose cord had not been provided with strain relief. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
