29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about March 17, 2015, on the west side of the commercial building, employees engaged in framing activities without fall protection were exposed to a fall hazard of greater than 6 feet.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided.    On or about March 17, 2015, on the west side of the construction site, the employer did not provide ladders for employees to access the upper level of a 3 story building under construction exposing the employees to a potential fall hazard of approximately 38 feet.
