29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated; the person certifying that the evaluation had been performed; and the date(s) of the hazard assessment.  A written certification of the workplace hazard assessment had not been developed.  Employees were provided and required to wear personal protective equipment such as hard hats, safety glasses, hearing protection, and chaps while trimming trees.
