29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.    Bldg 3857:  On or about 03/13/2015, employees working on a roof was not protected from a 12 foot fall hazard by the use of Personnel fall Arrest System or alternative protection.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    Bldg 3857:  On or before 03/13/2015, employer had not trained employees prior to beginning work on the 12 ft. roof.  Employees were exposed to fall hazards.
