29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about at 3/17/15 located at 4125 Williams Point Dr., Cumming, GA: Employees were framing a house with lumber and plywood on the roof and scrap plywood was observed falling to the ground as employees was working below passing the lumber up to the roof, without hard hats exposing them to a struck by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about at 3/17/15 located at 4125 Williams Point Dr., Cumming, GA:  Employees were exposed to19.5 falls and struck- by hazard, while preforming framing work from the make shift walking/ working surfaces. The employer failed to install guard rails and it was not fully decked.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about at 3/17/15 located at 4125 Williams Point Dr., Cumming, GA: Employees were installing lumber and plywood while standing on the roof of a house that was under construction working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 30 feet.
