(a) Charles Chapman Construction on or about March, 19, 2015, at the address of 1239 Shaffer Dr. Lorain, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifing and avoiding potential hazards such as falls and struck-by exposures during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Charles Chapman Construction on or about March, 19, 2015, at the address of 1239 Shaffer Dr. Lorain, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a residential type structure.  Employees operating in this condition were exposed to a 20 ft. fall to the ground below.
