29CFR 1926.501(b)(1):  Each employee on a walking/ working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by use of guardrail systems, safety net systems, or personal fall arrest systems:      a)  Employees engaged in installing new cellular antennas were not protected from fall hazards while working unprotected along the edge of a roof of a seven story building.

29 CFR 1926.501(b)(4):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels by personal fall arrest systems, covers or guardrail systems erected around such holes:      a)  Employees were not protected from the hazard of falling approximately 7.8 feet to the lower level while performing roofing work around three unguarded skylights.
