29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about March 19, 2015 Employees were exposed to fall hazards when the employer failed to ensure that a ladder used to access the roof of the structure extended at least three feet past the upper landing surface, the employer also did not ensure that the ladder used by employees was properly tied to the upper landing.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support.   On or about March 19, 2015 employees were exposed to fall hazards when the ladder that they were using was not properly angled to prevent the ladder from falling backwards when being used.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  On or about March 19, 2015 employees were exposed to fall hazards when the employer failed to remove a defective ladder from service and allowed employees to use when accessing the roof of the structure that was more than six feet in height.
