29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) On or about 3/20/15 at the Eden Heights Assisted Living Facility located at 4071 Hardt Road, Eden, New York; employees were installing a new torch down roof covering. There were approximately 10 skylights and an open roof hatch that employees were not protected from falling through.    NO ABATEMENT CERTIFICATION REQUIRED
