29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:   a)  On or about March 20, 2015 and at times prior thereto at the jobsite; there was a large amount of insulation, scrap wood, wood planks with nails protruding, and nails throughout the worksite where employees were working and walking.

The ladder was designed to be used in its open position.
