29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a.   On or about March 18, 2015, on the south side of the house, an employee installing asphalt shingles to a roof while using a pneumatic nail gun did not wear eye protection, exposing the employee to the hazard of being struck in the eye or face by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a.  On or about March 18, 2015, on the east side of a 9/12 pitch roof of a residential house, an employee working on the roof was not protected from a 10 feet 5 inch fall hazard by the use of a personal fall protection system.    Lowery Roofing Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 985486, citation number 01, item number 01 and was affirmed as a final order on August 7, 2014, with respect to a workplace located at 93 Dunes lake Circle, Santa Rosa Beach, FL 32459.
