At the site located on 4210 53rd Avenue East, Bradenton, FL, employees were performing sheathing work on a commercial 4/12 slope roof structure, while standing 15.0-ft. from the ground level without a fall protection system, on or about, 03/13/2015.

At the site located on 4210 53rd Avenue East, Bradenton, FL, an employee was engaged in the installation of trusses while standing on the plank of an outrigger scaffold system, 11.0-ft. above the ground level, without fall protection, on or about, 03/13/2015.
