29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:    Location: 159 Manchester Dr.    On or about: 12/22/14    a) Employees working in an excavation up to 12 ft. deep were exposed to cave in hazards. The employees were not provided with general safety and health training.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift:    Location: 159 Manchester Dr.    On or about: 12/22/14      a) Employees were working in an unprotected excavation that was up to 12 feet deep. A competent person did not perform a daily inspection.
