OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using an automotive lift:    (a)  On or about March 19, 2015, and at times prior the employer did not ensure that employees         were safe from crushing and struck-by hazards in that the following lifts were noted with deficiencies:        (1)  Rotary lift, model SPOA9-200, serial #SPA000D0812, was not lowered to rest on the locking mechanism while the             lift was raised and in use.             (2)  Rotary lift, model SPOA9-200 Serial #SPA00B3080, was not lowered to rest on to the locking mechanism while             the lift was raised and in use.            (3)  The employer did not document that all lift users were trained to use the lifts in that rotary lifts were not              lowered to rest on the locking mechanisms while in use.    (b) Auto lifts were not periodically inspected by a qualified inspectors at least annually to ensure that they were properly maintained and functioning.
