29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Second floor top plate, new residential construction located at 1 Hollow Way, Newnan, GA 30265: Employee working from the top plate at heights of approximately 19-feet, 5-inches engaged in framing activities was not protected from falls.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) First floor stairwell, new residential construction located at 1 Hollow Way, Newnan, GA 30265: Employees were exposed to falls of up to 10-feet, 3-inches to the concrete surface below while accessing a job made portable ladder which did not extend at least 3 feet past the second floor landing to allow employees safe access.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a) Front side, new residential construction located at 1 Hollow Way, Newnan, GA 30265: The employer failed to inspect and remove a 50 ft. 14 gauge flexible cord which had a torn and worn casing which exposed the internal circuits. This cord was used to power a Makita (Model - 5007F) 7-1/4-inch Circular Saw which was double insulated and plugged into a GFCI outlet.
