29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. On or about March 25, 2015, while performing trenching operations on Logan Street between E. 97th and E. 1012st the employer failed to provide a safe means of egress in the east trench that was approximately 9 feet deep.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about March 25, 2015, during the installation of sewer drainage line on Logan Street between E. 97th and E. 101st, Euclid Ohio 44123, the employer did not ensure that the excavated materials were placed at least 2 feet from the edge of the trench.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about March 25, 2015, during the installation of sewer drainage line on Logan Street between E. 97th and E. 101st Euclid Ohio, the employer did not ensure that employees were protected from a cave in hazard by using an adequate protective system.
