29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees working in the Boot Department were exposed to injury form unused portions of blades in that machine guarding was not adequate. The guard provided for the unused portion of the blade on the DoAll vertical band saw did not completely cover the blade, leaving 4 inches of exposure above the guard.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Employees working in the Liner Department were exposed to injury from a projectile hazard. The adjustable tongue guards for the abrasive wheels on the 6 inch, Skil, bench grinder, model 3380, were not installed/adjusted properly and would not have protected the employee in the case of an abrasive wheel failure.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): Employees in the Boot Department were exposed to injury from an in-going nip point hazard. The drive pulleys on the S & D machine, number 89, were not provided with guards on the right side and back of the machine. In-going nip points were created by two 4 inch pulleys and one 3 inch pulley.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): Employees working in the Boot Department were exposed to an electrical shock hazard due to exposed live parts of electrical equipment. The electrical control box housing 120 VAC energized conductors was not closed at the time of the inspection.
