29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Employees engaged in machining activities at the following pieces of equipment throughout the facility were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized:    a)  OTL Power Team 100 ton hydraulic press in the weld area, the hydraulic ram was not guarded at the point of operation.     b)  Bridgeport Mill in the machine shop, employees were not provided protection from flying chips and debris.    c)  Hunco Mill in the machine shop, , employees were not provided protection from flying chips and debris.    d)  Okuma machining center, workstation #21, in the machine shop, the electrical interlock on the door had been bypassed.    e)  Okuma machining center, workstation #23, in the machine shop, the electrical interlock on the door had been bypassed.    f)  Sun drill press in the blending area, the rotating drill chuck was not guarded.     Numerical Control Support, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1910.212(a)(1), which was contained in OSHA inspection number 450373, citation number 1, item number 1 and was affirmed as a final order on 10/16/2012, with respect to a workplace located at 21945 W. 83rd Street, Shawnee Mission, KS.
